The intent of this ERM format is to assist you in the completion of the MA ERM Form.
To use the MA ERM in Word format:
- Click on the 'MA ERM in Word format' link.
- Click on the FILE drop down menu and select SAVE AS.
- Select the appropriate drive and folder on your PC to save the ERM document in.
- Use the Tab key to navigate through the ERM document. (Do not use the Enter key.)
- Use the mouse to click on appropriate yes/no box responses within the document.
- When all information has been entered, you may wish to save the ERM as a different name for your records. This will also leave the ERM document blank for you to work from next time.
- After the ERM has been completed in Word, print the two page ERM document and have it signed by the employer.
- Submit the ERM to the Massachusetts Workers' Compensation Rating & Inspection Bureau and/or the current insurance carrier along with any other pertinent documents attached.
